Note: Green upload bar - the form has passed all validation and is ready for submission. Yellow upload bar - the form data and system information do not match while user may still submit the form if user considers correction is not necessary. Red upload bar - the form cannot pass validation and user is required to remove the form and make revision and upload the file again.
